The Schedule 1 form is designated for businesses or individuals who wish to register a retail shop or place where Cooked or Uncooked food items are sold, deposited, exposed or offered for the purpose of Human Consumption.
In filling out this online application, the business or individual recognizes that the application is made in pursuant of the bylaws made in that behalf of the place, hereinafter described as a Retail Shop or place for the sale, preparation, deposit, exposure or offer for sale of Cooked or Uncooked food.
